Reimbursement Request Form

CLICK HERE to complete and submit the Reimbursement Request form to get reimbursed for NHE PTA related expenses. (Note: If you have not yet created an account with this site, you will first be asked to setup your account).

  1. Go to the New Forms section and select PTA Reimbursement Form (see below):

  2. Then make a copy or take a picture of your receipts and email them to treasurer@nhepta.org  and disbursements@nhepta.org. Please make sure your reference your name on the receipts that you submit via email or hard copy.

  3. If the expense should come out of the disbursements account, please send your receipts to the Disbursements Chairperson at disbursements@nhepta.org or put them in the disbursements folder in the foyer cabinet, in the front hallway of the school.
