Reimbursement Request Form
CLICK HERE to complete and submit the Reimbursement Request form to get reimbursed for NHE PTA related expenses. (Note: If you have not yet created an account with this site, you will first be asked to setup your account).
- Go to the New Forms section and select PTA Reimbursement Form (see below):
- Then make a copy or take a picture of your receipts and email them to email@example.com and firstname.lastname@example.org. Please make sure your reference your name on the receipts that you submit via email or hard copy.
- If the expense should come out of the disbursements account, please send your receipts to the Disbursements Chairperson at email@example.com or put them in the disbursements folder in the foyer cabinet, in the front hallway of the school.
